Optimal Hospice Care can accept referrals from primary doctors, any healthcare professionals, or even family members like you.
WHO IS ELIGIBLE FOR HOSPICE CARE?
The patient has a serious, life-limiting or terminal illness
The burdens of aggressive curative treatment have started to outweigh the benefit of such treatment
The patient and family have decided that their goal is to maximize comfort and the quality of life
The patient's primary care physician has suggested hospice and agreed to write an order or evaluation for admission to the hospice chosen by the family
Optimal Hospice Care is dedicated to providing the utmost comfort care and dignity, through development of an individualized plan of care, developed by an Interdisciplinary Team and designed to meet the needs and personal choices of our patients and their families.
